At each visit you will check in with one of our specially trained patient account representatives who will verify and update your demographic information including address, phone numbers, insurance information and any other special billing requirements for that visit. Registration forms may be updated on a periodic basis. The DeKalb Clinic asks that patients pay for medical services, at the time they are rendered, by cash, personal check, or credit card. However, we do understand the challenges of multiple medical bills. Please contact our business office staff and let us help you set up an individualized payment plan for your specific circumstances. We will be happy to file your primary insurance automatically. This eliminates excessive paperwork for you and usually increases response time from your insurance carrier. We strive to process each claim accurately and efficiently. We are currently developing a system to file secondary insurance for all of our patients. At this time we will be happy to file your secondary insurance upon your request. We are the preferred provider for various HMO plans and other managed care programs and therefore have various administrative responsibilities. In some instances this may include being required to file insurance for your plan.
Patients with a Managed Care Plan may be required to obtain a referral from their Primary Care Physician if they need to see a specialist. While we will be glad to assist you to obtain file needed paperwork it is the patients responsibility to obtain this authorization PRIOR to being seen by the specialist. Failure to do so may result in reduction of benefits or denial of payment by your insurance carrier. Patients whose plans have a co-payment will be required to pay the designated amount at the time of service. We ask that Medicaid patients bring proof of their coverage at each visit. Medicaid patients may be required to obtain an referral from their Primary Care Physician before scheduling an appointment with a specialist. Any services denied by Medicaid for any reason will become the patient's financial responsibility. Our office will file all your Medicare claims for you. After receiving payment from Medicare, we will send you a statement showing the amounts you are responsible for including the 20% coinsurance, annual deductible and any non-covered services. If you have a supplemental insurance plan, once we receive payment from Medicare, we will also file your supplemental claim. At that time, we will send you a statement showing the coinsurance amount that will be due. Since your supplement carrier may send the check for the coinsurance to you, this statement will help you recognize payments that should be sent to our office.
